I accept Cottage Hospital Insurance (“Cottage Health”). Copay is due at each session.

I am not currently a Preferred Provider for any other insurance panels, however as an Out-of-Network Provider, I can provide monthly statements that you may submit to your insurance company with the possibility of receiving full or partial reimbursement, depending on your plan.
